problem with footer link

I had change the footer link and after that I had to purchase the free link version.

I pay 14.99 but now I still cant login in my wordpress because the link with purchase is still on the site.

after that I log in on this site went to the members area and saw this

Use form below to add link to the footer of template. IMPORTANT: after you save link parameters, you will not be able to change it.

but I dont see the form.
