How To Choose The Contact Form Template In A New Page

I´ve installed the theme Pokerfan and configured the Contact Form Framework (Email for messages etc.)

Then I´ve created a new page "Contact Us".

How/where do I choose now the "Contact Form" template to add it in the new page?
