Contact form displays errors even if the fields are filled

Hi I've some problem with the contact form here

1. it displays errors even if the fields are filled
2. it doesn't displays the map even if the google key is specified

where am I doing wrong?

the problem seems to be related with cloudflare and a particular setting of my host, I'm looking for a solution with their support.
